Pediatric Home Care Services 
​Wellspring Personal Care is licensed by the Illinois Department of Public Health to provide services for clients who require skilled nursing. We have extensive experience caring for fragile children with multiple chronic conditions, and have helped to maintain our young clients at home through adulthood. 

Wellspring Personal Care’s clinical management team includes a licensed clinical social worker, a registered nurse supervisor, and a physician who serves as medical director. We train our nursing field staff to understand the emotional impact of chronic illness or disability on normal family relationships (marital, siblings, extended family) and strive to match patients with nurses and aides who are sensitive to the unique needs of each patient and family. We prioritize working collaboratively with case managers and other health care providers, while engaging parents in problem solving and maintaining compliance with the plan of care. Our pediatric nursing field staff includes licensed practical nurses, and certified nurse assistants. 

A value-added component of Wellspring Personal Care’s service is Home Care Metrics℠, our proprietary web based program that can track, document and share clinical data. Home Care Metrics℠ chronic care management solution identifies changes in condition prior to the development of acute illness, and can prevent emergency hospitalization or 
re-hospitalizations.

Therapeutic Mentoring for Children with Disabilities
Wellspring Personal Care offers an innovative therapeutic mentoring program designed to bridge service gaps for children and young adults who have special needs. Mentors tailor intervention strategies to address the unique challenges faced by their clients whose disabilities or presenting problems fall in the mild to moderate range of emotional disorders, developmental delays or physical disabilities. 

Therapeutic Mentoring Activities focus on problems and issues related to life skill development, socialization/community integration, community service, tutoring, and recreational activities. We recognize that some clients may have health challenges, therefore the goal of this program is to improve the individual’s level of functioning in their living environment and broader community.  The program offers specialized attention for the identified client while providing the entire family much needed support and periodic respite.  

Our Team of Mentors come from several professional disciplines including social work, education, recreation therapy and mental health. Therapeutic mentors create systems of outreach and support so that recommendations for concrete services and enriching community based experiences are implemented. 

Mentors have experience working with children and/or young adults in a variety of settings, and have knowledge of child development models and behavior management techniques.

All mentors are screened, trained and undergo background checks. Mentors are employees of Wellspring Personal Care, and work in consultation with Wellspring’s management team.
